CONTRAINDICATIONSRisabafant is contraindicated in patients allergic toOxidic anticonvulsants such as pramipexole.
WARNINGAtazanavir is contraindicated in patients with epilepsy (seizures, epilepsy-like disorders), epilepsy-like disorders (fits), or severe epilepsy-like disorders (lasting between 10-30 days). Prior to initiating atazanavir, the patient should have a medical history and/or physical examination in order to evaluate for any unusual findings or symptoms. Should the seizure occur, the patient should have an immediate medical evaluation. During pregnancy, the patient should be treated with oral teratogenic drugs (anticonvulsants, antipsychotics, tricyclic antidepressants, selective serotonin reuptake inhibitors (SSRIs, dopamine receptor agonists, serotonin-norepinephrine reuptake inhibitors (SNRIs), tricyclic antidepressants, serotonin reuptake inhibitors (SRIs), benzodiazepines and anticonvulsants) to avoid any sedation or anxiolytic properties from the seizures. In addition, the patient should be observed for a few weeks after the seizure until seizure control. During the pregnancy, the patient should be observed for a few weeks after the pregnancy-related changes in the seizure pattern (moodiness, irritability, agitation) of the seizure. The patient should be monitored for several weeks after the pregnancy. The patient should be approached with care regarding the possible risk of fetal harm.
CAUSATIONPramipexole, a type of prescription anticonvulsant, should be used with caution in patients allergic to phenobarbital. Oxcarbazepine is an anticonvulsant and antipsychotic. Oxcarbazepine should be used only in patients in whom precipitation of the seizure occurs. Oxcarbazepine should not be given to a child with epilepsy and/or seizure disorders and who have ever had mania or depression (see also Drug Interactions).
Atazanavir should not be used in the setting of genetic polymorphic viral infections such as the herpes simplex virus type 1 (HSV-1) or herpes zoster (SZ). Herpes zoster (also known as varicella-zoster) is a late reactivant virus that causes chickenpox. Varicella-zoster is also a serious, occasionally fatal, condition that can occur in immunocompromised patients, including those with HIV. Atazanavir should be used only in patients in whom precipitation of the seizure occurs occurs in pregnancy.
DOSAGEThe usual recommended dose for treatment of patients in whom oxcarbazepine treatment has not been successful is 10 mg/kg once daily. The maximum recommended dosing frequency for treatment of patients in whom oxcarbazepine has been found not to be effective is 10 mg/kg/day. In the vast majority of oxcarbazepine-treated patients the efficacy of atazanavir is maintained at a dosage that reduces the risk of the adverse effects further. The risk of achieving a seizure reduction is greater in patients at high dosages (20 mg/kg/day). There are some patients in the dose range 50 mg/day who experience a seizure within the first 6 to 8 hours of starting treatment. Seizures have occurred in fewer patients treated with atazanavir (10 mg/kg/day) compared to patients treated with a lower dosage of atazanavir (0.5 mg/kg/day). The following are the most common side effects in these patients: headache, nausea, vomiting, palpitations (racing heartbeat), weight gain, loss of coordination, tremor (rigid feeling in the cervical region), nervousness, agitation, delirium, convulsions, convulsions, delirium often leads to coma, and seizures are more common in the lower dosage of oxcarbazepine (50 mg/day).
